PARENT QUESTIONNAIRE – Please complete this form after you register.
MCA invites parents (or guardians) to complete a parent questionnaire, and participation is voluntary. The parent questionnaire helps the Office of Multicultural Affairs (MCA) determine if we are reaching a diverse group of students during the Multicultural Youth Leadership Day at the Capitol. MCA invites you to complete the following form, because your child will attend the Leadership Day with their school. For parents who choose to participate, their child’s information remains confidential. The acknowledgement section is removed before any data collection, which means their responses do not identify them or their child’s name. We ask educators to collect the forms and provide them to MCA on the day of the event or email to rbenally-sagg@utah.gov. To print the Parent Questionnaire, click MYLD_Parent_Questionnaire.

If you have any questions regarding Registration, please contact Itzel Hernandez at itzelhernandez@utah.gov or 801-245-7282. Click on the link to obtain your own copy of this year’s flyer, MYLD_2017_Flyer.
